REGISTRATION FORM

Procedure:
1. You may download the Registration Form (25kb) (Right click and select "Save Target As...") and enter all your relevent particulars.

2. Next, fax the Registration Form to our office at 03-5633 6061.

3. Make your payment either by cheque (payable to RIGHT LIFESTYLE SDN BHD), by post or direct bank-in to our bank account in Maybank (A/C: 5124-8240-7173) or Public Bank (A/C: 399-2710-828)

4. We will send you a confirmation either by fax or e-mail.

Terms and Conditions Apply. No refund for cancellations but seats are transferable. The Organiser reserves the right to substitute venue and date.
